Before You Refer

  • Well & Plenty provides supportive residential environments rather than medical, nursing, assisted-living, behavioral-health treatment, or personal-care services.
  • Residential participants must be able to manage their own activities of daily living and independently manage and administer their own medications.
  • Participants must be able to live safely in a shared residential environment without continuous medical, clinical, or behavioral-health supervision.
  • A behavioral-health diagnosis alone does not make someone ineligible.
  • Well & Plenty does not provide behavioral-health treatment, crisis stabilization, medication management, ADL assistance, skilled nursing care, or continuous clinical supervision.

Submitting a referral does not confirm placement.
